In addition to the routine hygiene appointments, patients will receive a 20% discount on all other dental treatments with payment at the time of service with cash or check. This discount will drop to 17.5% if paid by credit or with a debit card and 10% if paid for by Care Credit due to merchant fees.

• Plans purchased are for that individual ONLY (no sharing – no transferring)

• The plan is for one year from the date of purchase- no benefit will be payable after termination date

• All services must be completed prior to the termination date

• This is a dental discount plan (not insurance) and cannot be combined with any other insurance or promotions

• To receive the discount, services must be paid for, in full, at the time of treatment

• Annual membership fees are subject to change at any time

• Membership fees are not refundable

• Children’s fees apply when the patient is under 14 years at the time purchase

• Discount applies only to services performed at Highland Park Family Dentistry and will not transfer to another office or specialist’s office
